Commitment Dr. Stephane Guzmán García protects your Protected Health Information (PHI). We maintain records to provide care and comply with the law. This notice explains how we use your PHI and your rights. We may update it; the current version will be in our office and at www.sggpractice.com. Uses Without Authorization • Treatment (coordinating your care) • Payment (billing) • Operations (quality improvement) • Legal or public health requirements (abuse, court orders, serious risks) • Appointment reminders and treatment alternatives. Uses Requiring Your Written Authorization • Psychotherapy notes (with clinical/legal exceptions) • Marketing or sale of PHI. You may revoke your authorization at any time, in writing. Disclosures with Opportunity to Object We may share minimal information with family members or people you designate for your care, unless you object. In emergencies, we may do so before we know your decision. Your Rights • Request restrictions on uses/disclosures (we can deny them if it affects your care). • Request that we not inform your insurance company when you pay out-of-pocket. • Choose how and where you receive communications. • Review and obtain a copy of your record (except psychotherapy notes) in ≤30 days; a reasonable fee may be charged. • Request an amendment; we will respond in ≤60 days. • Receive a list of disclosures not related to treatment, payment, or operations (past 6 years). • Obtain an electronic or paper copy of this notice at any time.
